Celeste Medina, PharmD

PGY1 Pharmacy Resident – Transitions of Care

Dr. Medina received her Bachelor of Arts in Applied Science and her Doctor of Pharmacy degree from the University of the Pacific. During pharmacy school, she worked as an intern at CVS pharmacy. She completed her rotations in the Inland Empire region of California at Riverside University Health System and Kindred Hospital Ontario.

Her professional interests include transitions of care, ambulatory care, and general pediatrics. During her free time, Celeste enjoys traveling, staying active, trying new coffee shops, and spending time with friends, family, and her dogs, Levi and Quincy.

Cezeelia Velasco, PharmD

PGY1 Pharmacy Resident

Dr. Velasco completed her prerequisite coursework at the University of Arizona in 2020 and earned her Doctor of Pharmacy degree at the University of Arizona R. Ken Coit College of Pharmacy in 2026. Throughout pharmacy school, she was involved in various student organizations such as Rho Chi Society, PediaCATS, APhA-ASP, IPSF, and PharmAssist Tutoring. She also worked as a pharmacy intern at Walgreens Pharmacy. She completed her APPE rotations in institutions such as Lucile Packard Children’s Hospital Stanford, Mayo Clinic Arizona, and Tucson Medical Center.

Her professional interests include general pediatrics and ambulatory care. In her free time, she likes to try new food spots in the city, travel, go to music festivals, read books, and most importantly, cuddle with her cat Churro.

Eilee Mendoza, PharmD

PGY1 Pharmacy Resident

Dr. Mendoza earned her Bachelor of Science in Human Biology from the University of California, San Diego in 2022 and her Doctor of Pharmacy degree from the University of California, San Francisco in 2026. During pharmacy school, she served as the lead pharmacy intern at UCSF Benioff Children’s Hospital. She completed her APPE rotations in the Greater Sacramento region, including rotations in the PICU and NICU at UC Davis Health. She also volunteered as a case manager with Clínica Tepati, a student-run nonprofit clinic that caters to the underserved Latinx community in Sacramento.

Her professional interests include pediatric critical care and neonatal intensive care. In her free time, she enjoys thrifting, exploring new coffee shops, going on long scenic walks, and attending comedy shows with her friends! 

Vicki Tran, PharmD

PGY1 Pharmacy Resident

Dr. Tran earned her Bachelor of Arts degree in Molecular & Cell Biology with an emphasis in Immunology and Molecular Medicine in 2023 at the University of California, Berkeley and her Doctor of Pharmacy degree at the University of California, San Francisco in 2026. During pharmacy school, she worked as an inpatient pharmacy intern at Kaiser Permanente and a discharge pharmacy intern at Santa Clara Valley Medical Center.

Her professional interests include general pediatrics and ambulatory care. In her free time, she enjoys trying coffee and matcha shops, going to concerts, collecting cameras, crafting, and exploring new cities with friends.

Jeremiah Samson, PharmD

PGY1/PGY2 Combined Program Health-System Pharmacy Administration Leadership Resident

Dr. Samson earned his Bachelor of Science in Pharmaceutical Sciences from the University of California, Irvine in 2020, followed by a post-baccalaureate certificate from the University of California, San Francisco (UCSF) in 2021 and his Doctor of Pharmacy from UCSF in 2026. He is currently pursuing a Master of Science in Pharmaceutical Sciences, specializing in Pharmacy Administration and Leadership, through the University of North Carolina at Chapel Hill as part of a combined PGY1/PGY2 HSPAL residency program. During pharmacy school, he worked as an inpatient pharmacy intern at UCSF Medical Center and completed his APPE rotations in Fresno, California, including PICU and NICU rotations at Valley Children's Hospital.

His professional interests include pediatric critical care, pharmacy administration, and clinical informatics. Outside of work, he enjoys cooking and trying new foods, watching shows and movies, reading, playing volleyball and pickleball, board and video games, and spending time with friends.

Julia Halloran, PharmD

PGY2 Pediatrics – Critical Care Pathway

Dr. Halloran completed her prerequisite coursework at the University of Arizona and earned her Doctor of Pharmacy degree from the University of Arizona R. Ken Coit College of Pharmacy. During pharmacy school, she worked as a pharmacy intern at Phoenix Children’s Hospital and Tucson Medical Center. She completed her student rotations at various sites in multiple states, including Lucile Packard Children’s Hospital Stanford, Texas Children’s Hospital, and Phoenix Children’s Hospital. She completed her PGY1 residency training at Lucile Packard Children’s Hospital.

Her professional interest is in pediatric critical care. In her free time, she likes to travel, spend time with loved ones, read, and go on coffee runs.

Nick Ha, PharmD

PGY2 Pediatrics – Critical Care Pathway

Dr. Ha received his Bachelor of Arts degree in Applied Science and his Doctor of Pharmacy degree from the University of the Pacific. During pharmacy school, he was involved in promoting professional advancement, education, and mentorship with the California Society of Health-System Pharmacists. He completed his APPE rotations in the San Diego region, including rotations at Rady Children’s Hospital. He completed his PGY1 pharmacy residency at Lucile Packard Children’s Hospital Stanford.

His professional interests include critical care and cardiology. During his free time, Nick enjoys longboarding, swimming, visiting museums and aquariums, trying new restaurants, and watching horror movies.

Joseph Miles Azurin, PharmD

PGY1/PGY2/MS Combined Program Health-System Pharmacy Administration Leadership Resident

Dr. Azurin completed his Bachelor of Science in Pharmacy at the University of the Philippines and received his Doctor of Pharmacy Degree from Loma Linda University. He is also currently in his final year pursuing a Master of Science degree in Pharmaceutical Sciences specializing in Pharmacy Administration and Leadership through the University of North Carolina at Chapel Hill as part of the PGY1/PGY2 combined HSPAL program. As a student pharmacist, he was recognized and awarded nationally and internationally for his leadership and initiatives through the American Pharmacists Association and the International Pharmaceutical Students’ Federation.

Dr. Azurin’s professional interests include informatics, technology, and medication safety. In his free time, he enjoys playing guitar and designing computer graphics.
